AORTOBIFEMORAL BYPASS GRAFT

Indication: Severe aortoiliac occlusive disease causing incapacitating claudication, rest pain, or limb-threatening ischaemia, where endovascular options are not feasible or have failed. Five-year patency >90%.

Patient Position: Supine.

Anaesthesia: General (with or without epidural). Urinary catheter and arterial line placed.

Incision: Midline laparotomy for aortic access. Bilateral vertical groin incisions over the femoral vessels.

Procedure:
1. Abdomen entered. Retroperitoneum opened over the aorta. Infrarenal aorta isolated.
2. Both femoral arteries exposed in the groins: common femoral, superficial femoral, and profunda femoris arteries controlled.
