OPERATION: Excision of Thyroglossal Cyst (Sistrunk's Procedure)
ANAESTHETIC: General endotracheal anaesthesia
POSITION: Supine with neck extended

INDICATION:
Thyroglossal cyst / recurrent thyroglossal cyst. [Size, symptoms, previous infection].

PROCEDURE:
WHO Checklist performed. Transverse skin crease incision over the cyst, centred in the midline. Dissection deepened through platysma. Cyst identified in the midline of the neck, closely associated with the hyoid bone.

Cyst dissected circumferentially. A core of tissue including the tract is traced superiorly. Central portion of hyoid bone excised [approximately 1 cm of the central hyoid body]. Dissection continued superiorly through the base of tongue musculature, following the tract to the foramen caecum, taking a core of muscle with the specimen to reduce recurrence.

Tract suture-ligated at the level of the foramen caecum. Specimen excised.

Wound irrigated. Haemostasis confirmed. Strap muscles approximated over the defect in the hyoid. Platysma closed. Skin closed with subcuticular absorbable suture.

FINDINGS:
Thyroglossal cyst [size, contents — clear / mucoid fluid]. Tract traced to [level].

SPECIMEN: Cyst and tract to histopathology.
EBL: Minimal. COMPLICATIONS: None.

POST-OPERATIVE INSTRUCTIONS:
Regular analgesia. Soft diet for 48 hours. Sutures self-dissolving. Outpatient review with histology.
