Preparation and Position:
Sign in; who checklist completed, GA, supine, prepped and draped
Incision and Approach:
Vertical midline scrotal incision to gain access to both hemiscrotum
OR left and right transverse hemi- scrotal incisions

Procedure:
Dissection through scrotal layers with bipolar diathermy. Reactive hydrocele upon opening, testis delivered. Testis torted ...... degrees, dusky, untwisted and wrapped in warm saline gauze, proceeded to fix right side. Right side: Dissection through scrotal layers with bipolar diathermy. Testis delivered looks healthy, No hydatid cyst noted. Subdartos pouch created. Testis fixed with 5/0 prolene (3 points; inferior, medial and lateral). Correct orientation checked prior to fixation, lateral sulcus laterally superior pole superiorly. Good haemostasis. Went back and reassessed the left testis. Cord and epididymis pinked up nicely. Testis improved but still dusky. (Given duration of history ~ 6 hrs and the improvement of the prefusion), decision was to fix the testis. Testis fixed with 5/0 prolene (3 points; inferior, medial and lateral) correct orientation checked prior to fixation, lateral sulcus laterally superior pole superiorly. Good haemostasis.
Closure: Dartos; 4/0 vicryl interrupted, Skin; 5/0 Vicryl Interrupted. Opsite spray
