OPERATION: Laparoscopic Totally Extraperitoneal Patch Plasty (TEPP) Repair of Incisional Hernia
ANAESTHETIC: General anaesthesia

FINDINGS: Incisional hernia at [site]. No femoral hernia.

PROCEDURE:
12mm port placed in retrorectal plane. Retrorectal space created.
2 midline 5mm ports placed under direct vision.
Inferior epigastric artery and round ligament identified.
Fascial defect identified and sac reduced.
No femoral defects seen.
Sufficient space created for mesh placement.
15 x 15cm mesh trimmed and placed covering defect and deep ring.
Mesh kept in situ during desufflation. Ports removed.

CLOSURE:
J-PDS to 12mm port site anterior sheath.
3-0 Monocryl to skin. 40ml 0.25% levobupivacaine infiltrated.

POST-OPERATIVE INSTRUCTIONS:
Eat and drink as tolerated. Home same day if well.
No heavy lifting for 8 weeks.
