OPERATION: Laparoscopic Extended Right Hemicolectomy
ANAESTHETIC: General anaesthesia

FINDINGS: Tumour in transverse colon. No peritoneal disease.

PROCEDURE:
Mobilisation of right colon with protection of duodenum.
Ileocolic pedicle, right and middle colic vessels divided.
Lesser sac entered. Proximal transverse colon mobilised beyond middle colics. Splenic flexure mobilised.
Distal ileum and transverse colon divided with Echelon Flex 60 Blue.
Side-to-side stapled isoperistaltic ileocolic anastomosis.
Continuous closure of enterotomy with 3-0 PDS. Crotch stitch 3-0 PDS.
Mesenteric defect closed. Suction irrigation. Haemostasis ensured.

CLOSURE:
No.1 PDS continuous fascial sutures. 3-0 Monocryl to skin.

POST-OPERATIVE INSTRUCTIONS:
Mobilise. Analgesia. Incentive spirometer. Chest physiotherapy.
LMWH prophylaxis for 4 weeks.
Sips of clear fluids today, progress as tolerated.
