OPERATION: Laparoscopic Sigmoid Colectomy
ANAESTHETIC: General anaesthesia

FINDINGS: Lesion in distal sigmoid with tattoo noted distally.

PROCEDURE:
Pneumoperitoneum via Hasson technique.
2x 5mm ports in left iliac fossa and right mid-abdomen.
3x 10mm ports in umbilical, right iliac fossa and suprapubic region.

IMA identified with preservation of ureters and pelvic nerves. IMA divided.
Upper rectum mobilised. Upper mesorectum divided with Ligasure.
Rectum divided with Echelon stapler 60.
Mesentery divided to point of convenience.
Midline utility incision. Resection completed.
Colorectal anastomosis fashioned using 29mm CDH circular stapler.
Complete doughnuts confirmed. Negative air insufflation test.
Haemostasis confirmed.

CLOSURE:
Fascia closed with PDS. 3-0 Monocryl to skin.

POST-OPERATIVE INSTRUCTIONS:
Sips orally as tolerated. Analgesia. VTE prophylaxis.
Monitor progress. Chest physiotherapy.
