OPERATION: Laparoscopic Appendicectomy with Pelvic Drain
ANAESTHETIC: General anaesthesia
ANTIBIOTICS: Co-amoxiclav given at induction

WHO Checklist performed.

FINDINGS: Inflamed appendix with free fluid in pelvis.

PROCEDURE:
Open Hasson technique. Supraumbilical 10mm port. Left iliac fossa 5mm and suprapubic 5mm ports under direct vision.

Appendix dissected off mesoappendix. Clips to appendicular vessels.
Endoloops x3 PDS on appendix base, 2 to stay.
Appendix divided at base and brought out using retrieval bag.
Suction irrigation performed. Haemostasis ensured.
20F drain placed in pelvis and secured.

CLOSURE:
No.1 PDS fascial sutures to umbilical port.
3-0 Monocryl subcuticular sutures to skin. Dressings applied.

POST-OPERATIVE INSTRUCTIONS:
Dalteparin at 18:00. Drain emptied at 08:00 daily.
Bloods the following morning.
Continue antibiotics total 5 days (48 hours IV, 72 hours oral).
