OPERATION: Laparotomy and Adhesiolysis
ANAESTHETIC: General anaesthesia

FINDINGS: Band adhesion causing internal hernia. Viable bowel following release. No ischaemic bowel.

PROCEDURE:
Midline laparotomy.
Bowel adherent to anterior abdominal wall released.
Band adhesion ligated and divided.
Bowel freed from adhesions.
Small bowel run from duodenojejunal flexure to ileocaecal valve.
Serosal injuries repaired with interrupted 3-0 PDS.
Abdominal viscera inspected and found normal.
Peritoneal fluid sent for MC+S.
Wound catheters placed bilaterally.

CLOSURE:
Sheath closed with x2 looped PDS. Clips to skin.

POST-OPERATIVE INSTRUCTIONS:
Nasogastric tube on free drainage. Progress to oral diet when outputs reduce.
LMWH prophylaxis. Clip removal at 2 weeks.
