OPERATION: Laparoscopic Ventral / Incisional Hernia Repair (IPOM — Intraperitoneal Onlay Mesh)
ANAESTHETIC: General anaesthesia
ANTIBIOTICS: IV antibiotics at induction. [Bowel prep if colon expected in sac.] VTE prophylaxis. Urinary catheter.
POSITION: Supine.

INDICATION:
Ventral / incisional hernia — symptomatic or irreducible. [Primary / recurrent]. Laparoscopic approach chosen after discussion of open alternative. Previous operations reviewed and planned access sites selected to avoid adhesion-prone areas.

PROCEDURE:
WHO Checklist performed. Full abdomen prepared.
