OPERATION: Reversal of Ileostomy (Restoration of Bowel Continuity)
ANAESTHETIC: General anaesthesia
CEPOD: Elective
ANTIBIOTICS: Given at induction
INCISION: Circumferential incision around ileostomy.
PROCEDURE:
Dissected all round ileostomy up to fascia.
Adhesiolysis by blunt and sharp dissection.
Dissected to where terminal ileum was attached to proximal ascending colon.
Segment approximately 10cm excised and sent for histology.
Adequate length of ileum and colon cleared for anastomosis.
Enterotomies created on both limbs. Stapled anastomosis 5cm performed.
Enterotomy closed in 2 layers. Haemostasis ensured. Instrument count confirmed.
CLOSURE:
No.1 PDS interrupted sutures to fascia. Wound irrigated. Collatamp applied.
2-0 Vicryl subcutaneous sutures. 3-0 Monocryl subcuticular sutures to skin.
POST-OPERATIVE INSTRUCTIONS:
Clear fluids orally today. Bloods the following morning.
Dalteparin at 23:00. IV Metronidazole as prescribed.
