OPERATION: Laparoscopic Removal of Gastric Band and Port
ANAESTHETIC: General anaesthesia

FINDINGS: Gastric band in situ. No evidence of gastric wall erosion. No port site hernias. Band adherent to undersurface of left lobe of liver.

PROCEDURE:
Visiport entry at left paramedian point.
5mm left upper quadrant and 10mm right upper quadrant ports under vision.
Gastric band removed with hook diathermy to divide capsule and simple traction.
Band removed through 10mm port. Haemostasis maintained throughout.
Previous port placement site entered to retrieve port and tubing. Fascia intact.
Adjacent 10mm port site fascia closed superficially with 0-PDS on J needle.
2-0 Vicryl subcutaneous interrupted sutures.
3-0 Vicryl subcuticular skin closure. Levobupivacaine infiltrated.

POST-OPERATIVE INSTRUCTIONS:
Clear fluids, then build up to free fluids and soft diet.
Regular analgesia. Home after recovery. No follow-up required.
