OPERATION: Laparoscopic Cholecystectomy
ANAESTHETIC: General anaesthesia
CEPOD: Elective

WHO Checklist performed. Prep and drape in sterile fashion.

FINDINGS: [Describe gallbladder - e.g., chronically inflamed, containing calculi].

PROCEDURE:
Supraumbilical open Hasson 10mm port.
10mm epigastric, 5mm right subcostal and 5mm right lumbar ports under direct vision.

Correct identification of anatomical structures.
Cystic duct and cystic artery identified and dissected. Critical view of safety achieved.
Hem-o-lok clips applied - doubly to cystic duct proximally, once distally; divided.
Cystic artery similarly clipped and divided.
Cholecystectomy performed in standard fashion.
Haemostasis and biliostasis confirmed in surgical bed.
Washout with normal saline. Suction irrigation.
Gallbladder brought out using retrieval bag via umbilical port.

CLOSURE:
No.1 PDS sutures to fascia at umbilical and epigastric ports.
3-0 Monocryl subcuticular sutures to skin. Dressings applied.

POST-OPERATIVE INSTRUCTIONS:
Eat and drink as tolerated. Regular observations.
